How Do I Get a Sick Note?

For the first 7 calendar days of an illness, you do not need a sick note from a healthcare professional; you can self-certify your absence with your employer.

If you are still unwell and unable to work after 7 days, you will need an official sick note from the healthcare professional most involved in your care.

A sick note can say that you are not fit for work or may be fit for work, with suggested adjustments where appropriate.

When your sick note expires and you feel well enough to return, you do not need another note confirming that you are fit for work.

What Should I Do for the First 7 Days?

Self-certify your absence directly with your employer. Your employer may have its own form for this.

Who Should I Ask for a Sick Note?

If you are still unwell and unable to work after 7 days, request a sick note from the healthcare professional most involved in your care. This could be Blithehale Medical Centre, a hospital doctor, physiotherapist, practice nurse or pharmacist managing your condition.

What Can a Sick Note Say?

  • Not fit for work.
  • May be fit for work, with suggested adjustments such as a phased return, different hours or amended duties to help you and your employer discuss a safe and gradual return.

If Your Sick Note Is About to Expire

If you do not feel ready to return to work, contact the practice for a review before your sick note runs out.

Returning to Work

When your sick note expires and you feel well enough to return, you do not need a note from the practice saying that you are fit to return. You are assumed to be fit for work.
